WPADE offers a scholarship to those who have successfully passed the CDE certification or recertification examination, and are not getting reimbursed.
An application is only eligible to win the award one time. Applicants must also have served on a committee, as a board member or assisted in the planning of a program for WPADE within the past 3 years to qualify for this scholarship.
To be considered for the scholarship please forward -
- Scholarship Application (Note: Scholarship Application opens in a new window)
- a copy of your form of payment for the examination
- a signed statement veryifying that your place of employment or other affiliates are not providing reimbursement for the examination fee
- proof of membership in WPADE
- a copy of your results showing that you successfully passed the examination
- and a completed copy of this application to:
